Heart of Gold Homecare LLC provides mentorship and peer support services for all the participants residing in our home care facility. We create connections between our participants and their communities, exploring resources and referral networks and facilitating inclusion.
Our mentorship services aim to enhance and encourage self-advocacy and community living among our participants in issues related to community living and real-world experiences. Our mentorship as one of our supported living services:
- provide our participants with advice
- model successful problem solving
- help with meeting future providers
- provide education about complicated health and safety issues
- complete and review safety assessments
- provide company to a participant to his or her therapy session to help with speaking with healthcare providers
- help with subsequent follow-ups and in discovering resources inside the community and family
